Patients receiving services from Community Care of Wake and Johnston Counties (CCWJC) have the right to:

  • Ask for information about programs and services
  • Receive information that is easy to understand
  • Receive communication specific to any vision, speech, hearing, cognitive and/or language-impaired patient, as appropriate to the impairment
  • Ask about CCWJC staff and their qualifications
  • Know the staff members that will provide their care management services
  • Know who to ask to change care managers
  • Make decisions about their own health care
  • Receive information about all care management-related services available to them, even if a service is not paid for by Medicaid
  • Discuss service options with their physician(s) and other medical providers
  • Have personal and medical information kept confidential, unless disclosure is permitted by law.
  • Be told who has access to their information upon request
  • Know how CCWJC ensures security, privacy and confidentiality
  • Be informed about any contract relationships
  • Be treated with courtesy and respect by CCWJC staff
  • Make a decision not to participate in programs offered by CCWJC
  • Be instructed on how to file a complaint with CCWJC

    Patients receiving services from CCWJC have the responsibility to:

  • Listen to the advice offered by CCWJC staff
  • Give staff correct and complete information so they can provide the right services
  • Notify CCWJC if they decide to stop participating in the program
