Four Community Care of Wake and Johnston Counties practices are part of 39 practices statewide involved in a demonstration projects to integrate mental health and physical health providers. Three practices will integrate a mental health provider into the primary care practice. One practice will integrate a physical health provider into a mental health practice. These projects are intended to improve access to mental and physical health services in high Medicaid practices to improve care for this population.
