Program Overview
|
Community Care of North Carolina (CCNC) (formerly known as Access II and III) is building community health networks organized and operated by community physicians, hospitals, health departments, and departments of social services.
Community Care of Wake and Johnston Counties (CCWJC) is one of the 15 Community Care of North Carolina networks across the state. CCWJC focuses on population management for medically high cost and high risk Medicaid patients. Disease Specific Initiatives promote adherence to best practice guidelines for chronic diseases. Pharmacy Initiatives promote cost effective prescribing of medicines, patient adherence to medicines, and patient safety. Access to Primary Care Initiatives promote access to primary care and seek to limit non-emergent use of Emergency Departments. Case Management Services provide direct case management services to high risk, high cost patients and support to primary care practices.
Announcements:
- Open Provider Lists: If a practice wants to open to new Carolina Access Medicaid patients, they should call DMA provider services at (919) 855-4053. The state level DMA services will then notify the county level Carolina Access/Managed Care Representative. They in turn will notify Community Care of Wake and Johnston County and we will update our Open Provider Lists. If you have any questions or notice that your practice is open to new Carolina Access Medicaid patients, but not on our Open Provider List, please contact Tina Sullivan at 792-3644.
- Case management update:
CCWJC case management staff have begun to see patients in area hospitals and assist in the planning for the transition to home. We are currently working at the WakeMed New Bern Avenue campus and remotely with Rex Hospital. We begin the planning process with the discharge planner and patient in the hospital, meet the patient as an inpatient and plan for a follow up visit within a few days of discharge. At this visit we have the copy of the discharge instructions and make sure the patient is linked to services, has filled all prescritions and is linked back to their Primary Care Provider.
We always welcome referrals on any of your Carolina Access II patients. Any patient who would benefit from additional patient education, disease management (particularly asthma, diabetes, congestive heart failure), discussion about medication or dietary compliance should be referred to a case manager for intervention. Use the attached form to make the referrals.
