Pharmacy

The Community Care Program of Wake and Johnston Counties is one of 16 community networks under Community Care of North Carolina.  CCWJC has created private and public partnerships to improve management and delivery of care for Carolina ACCESS Medicaid recipients.  Pharmacists have recently been added to the mix to facilitate growth and implementation of network initiatives with an expertise and perspective in pharmacy utilization untapped to date within CCNC. 

The Network Pharmacist assists with the development, implementation and evaluation of pilot initiatives.
They work closely with Case managers and assist with delivery of chronic care management in the areas of diabetes, asthma and congestive heart failure with concentration on intensive medication therapy management review.  Most importantly, network pharmacists develop meaningful relationships with partners (prescribers, case managers, progressive pharmacist) so that communication of drug information and expertise is efficient and effective.
