Pharmacy Home

The primary goal of the Pharmacy Home Project is to create a “Pharmacy Home”, virtual or otherwise, where drug use information from multiple sources is gathered to better inform prescribing and intervention strategies.
CCNC Network Pharmacist use the pharmacy home computer based software program to exchange patient information between case managers, physicians, and local pharmacist within the community.
It is a web-based application that serves as a data source for addressing medication related problems within the Medicaid community such as polypharmacy and poly- prescriber related issues. 
 The pharmacy Home program has 3 criteria for enrollment:

  • ≥ 24 drugs in 3 months
  • Visits to > 3 medical practices in 6 months
  • Enrollment requests by prescribers, case managers or network pharmacist

Pharmacy Home is available to network prescribers, case managers and retail pharmacists.  It will store and organize community pharmacy Medicaid claims data, FORM (focused risked management) pharmacists notes, and other medication or Medicaid policy related information.
